Current Dental License 
  Copy of current dental license 
  Record of disciplinary action may affect candidacy for membership

Practice Specialty (if applicable)
  Specialists must provide copy of specialty certificate

Sedation Authorization (if applicable)
  Copy of permit showing authorization for you (or the 
  anesthesiologist)

Malpractice Insurance
  Copy of current documentation of adequate malpractice
  insurance

Once you’re a member, your practice must continue to meet our standards with ongoing screening.

Ongoing Screening Includes:

• Routinely verifying state dental license and malpractice renewals
• Sending a reminder to you prior to your license and malpractice expiration
• Reviewing patient feedback
• Consumer input occasionally leads to a closer review of a member office

Please note: Our primary goal is to protect the integrity of our membership base and the safety of our callers. All information submitted on your application will be checked for accuracy. Your application information will be held in the strictest confidence. All claims of membership and status within industry organizations such as the American Dental Association (ADA) will also be verified.
